Behavioral health

Some health conditions can change your mood and behavior. Learn about different kinds of behavioral and mental health conditions, symptoms and treatments, and find out what resources, programs and services are available.

Case Manager

If you have a major illness, injury or chronic health condition, you may be assigned a case manager. They will help you:

  • Better understand and follow your treatment plan, condition or disease
  • Find in-network providers and behavioral health support
  • Set up doctor visits and follow-up appointments
  • Answer questions between doctor visits

A case manager may be assigned to you based on your needs. Call Member Services with any questions.

When you need a little extra help, it can be hard to know where to go. Use our Community Resource Link to find free or low-cost services near you.

Health self-assessment

Complete the online health assessment so we can connect you to programs that can help manage your care. You will receive a personal report that describes how risk factors can affect your health.

Translation and interpretation assistance

Get an interpreter for your doctor appointment if English is not your first language.

BlueRide Transportation service

BlueRideSM offers you a safe and reliable ride to covered medical, dental and pharmacy visits with providers in your plan’s network.

You can also use BlueRide to go to participating fitness locations, health and wellbeing classes, and Narcotics Anonymous and Alcoholics Anonymous meetings.1

Care Coordinator

A Care Coordinator is a licensed nurse, social worker or qualified healthcare provider who can help you:

  • Access home healthcare, community-based services and local resources
  • Understand your benefits and help get you enrolled in special programs
  • Find transportation to doctor appointments
  • Coordinate care between your primary care clinic and specialists
  • Manage your care before and after a hospitalization

Your Care Coordinator will typically reach out to you within the first month of joining the plan. If you have not been contacted by a Care Coordinator, call Member Services with any questions.

Nurse line

Registered and experienced nurses are available 24 hours a day, 7 days a week when you need medical advice or guidance on where to go for care.

Our nurses can also help you during or after your pregnancy.

Develop and maintain a quit smoking or vaping plan with support from a wellness coach.

Blue Advantage Families and Children

County car and booster seat program

Blue Plus works with counties and other agencies statewide to provide car and booster seats to eligible members.

Children under the age of 9 as well as pregnant Blue Plus members are eligible for this benefit.

Eligible members living in Anoka, Dakota, Hennepin, St. Louis, and Blue Earth counties can contact Everyday Miracles directly.

All other eligible members can contact their local County Public Health office.

Healthy rewards incentive program

When you choose to join Healthy Rewards, you and your family can earn rewards when you complete certain preventive screenings and healthy activities.

Learn to Live

Living with stress, insomnia, depression, substance use or social anxiety? Learn to Live is an online program that's available anytime to help you work through it.

Omada

You can get healthier, lose weight, and prevent or manage diabetes or high blood pressure with Omada’s online programs.

These programs use heath coaches, connected devices, and lessons made just for you to help you build healthy habits for life.

SecureBlue (MSHO)

Animatronic pets

For members with a cognitive impairment diagnosis, animatronic pets can help provide comfort and companionship and improve mood. Choose from a cat, dog or bird.2

Friendly helper

Get up to 48 hours per year of in-person and virtual support services to increase community connections and help with everyday tasks such as light household chores, grocery shopping and more.

Living with stress, insomnia, depression, substance use or social anxiety? Learn to Live is an online program that's available anytime to help you work through it.

Medication therapy management

You can get free comprehensive medication reviews through the Medication Therapy Management (MTM) program as part of your plan. A specialized pharmacist will:

  • Talk about your list of medications and how you take them
  • Discuss ways to manage any side effects that bother you
  • Look for ways to lower your costs

After the review, you’ll get a written summary of what you talked about, and a recommended to-do list with specific suggestions.

If you are taking multiple medications, a specialized pharmacist can meet with you to talk about your list of medications, find ways to manage any side effects that bother you and look for ways to lower your costs. Receive a $50 reward card when you complete a comprehensive medication review.4
 

Senior Linkage Line

The Senior LinkAge Line, a service of the Minnesota Board on Aging, offers free statewide information and resources.

Disability hub

Disability Hub MN is a free statewide resource network that helps people with disabilities solve problems, navigate the system and plan for the future.

Household Support (myFlexCard)

Get help paying your utility bills and rent with a quarterly allowance of $260. Eligible members will receive their myFlexCard separately. This reloadable card can be used to pay for rent and utilities such as electricity, water, sanitation, phone, and cable bills.3

Music Therapy

Enhance your quality of life, improve physical and emotional health, and reduce stress with music therapy.

Members living in an assisted living or skilled nursing facility or foster home and experiencing certain mental health-related needs are eligible to receive up to 26 sessions per year with a board-certified music therapist.1

SilverSneakers® is an opportunity to improve your health, gain confidence and connect with your community. Stay active with fitness classes for ages 50+, on-demand workout videos and access to a broad network of fitness locations.
