Plan overview

SecureBlue (HMO SNP) is a Minnesota Senior Health Options (MSHO) plan that combines your Medicare and Medical Assistance into one plan. 

You must qualify for Medical Assistance, be 65 or older, and have both Medicare Part A and Part B to qualify for SecureBlue.

Medicare and Medical Assistance are programs that help you pay for healthcare.

SecureBlue is available in all 87 Minnesota counties.

There is no monthly premium for this plan. For 2025, members will have $0 copay for prescription drugs.

SilverSneakers® is an opportunity to improve your health, gain confidence and connect with your community. Stay active with fitness classes for ages 50+, on-demand workout videos and access to a broad network of fitness locations.

Care Coordinator

A Care Coordinator is a licensed nurse, social worker or qualified healthcare provider who can help you navigate your plan and healthcare. 

Your Care Coordinator will typically reach out to you within the first month of joining the plan.

Appointing a representative

You may name a family member, friend, doctor or anyone else to represent you. If you want to appoint someone who is not already authorized under state law, you and that person must complete Medicare's Appointment of Representative form. 

Rights and responsibilities upon disenrollment

When you leave or end your membership in SecureBlue (HMO SNP) it's called a disenrollment. A voluntary disenrollment is when you decide to leave the plan. You can leave the plan at any time. Leaving our plan does not impact your eligibility for Medicare and Medicaid (Medical Assistance). An involuntary disenrollment is when the plan must end your membership. There are limited situations when you may not choose to leave, but we are required to end your membership. An example of this is if you provided fraudulent information on your enrollment form or allow abuse of your enrollment card.

If you are disenrolled for any reason, the plan will notify you of the date your membership will end and the reason for it ending. If you have questions about disenrollment, please contact Members Services at 1-888-740-6013, TTY 711.
