Interested in enrollment?
Complete the Open Enrollment Change of Coverage form found in the retiree booklet mailed to you before Open Enrollment. Mail or fax the completed form to the U of MN Office of Human Resources. The University of Minnesota will notify Blue Cross to mail an enrollment kit to your home. The enrollment kit will include the required enrollment forms to enroll in coverage.
Contact information is located on the open enrollment form. You will receive additional enrollment forms from Blue Cross Blue Shield of Minnesota at your home address.
Plan 1: Complete the enrollment forms for both the U of M Retiree Plan 1 and Group MedicareBlue Rx.
Plan 2: Complete the Medicare Supplement Enrollment form and the Group MedicareBlue Rx enrollment form.
Mail your completed enrollment form[s] and the copy of your Medicare card in the postage paid envelope provided in your kit.
New to Medicare
- If you are actively working and covered by your employer's group plan, you may defer enrolling in Medicare until you stop working.
- If you are retired prior to turning 65, you should enroll in Medicare 3 months prior to your 65th birthday month so that it is active the first of the month in which you turn 65.
