Common life events that may qualify for special enrollment include:

  • Job loss
  • Early retirement
  • Loss of dependent status
  • Marriage
  • Divorce
  • Birth or adoption of a child
  • A move

You'll need to enroll within the required time frame — usually within 60 days of the life event.

You can fill out and send the completed and signed application form, payments and documents via one of the options outlined below.
