There are three versions of the 1095 (A, B and C). The one you get depends
on how you get your health insurance.
You’ll generally get Form 1095-B by mail from Blue Plus in February.
On your own from MNsure
You’ll get form 1095-B by mail from the Minnesota Department of Human Services.
You’ll get Form 1095-B or Form 1095-C (or both), depending on the size of your employer and whether it provides fully insured or self-insured coverage.
Please call the customer service number on the back of your member ID card or, if you have coverage through your employer, contact your human resources department.
