Authorizing a Personal Representative

 

Medicare Advantage and Minnesota Health Care Programs members can choose to let a Personal Representative use a third-party health application for them. Here's how you can make that happen.
 

How to enable Personal Representative access

Please follow these steps to let a Personal Representative access a third-party health application for a Blue Cross and Blue Shield of Minnesota (Blue Cross) member.

Step 1. Create an account

First, the representative needs to have an online Blue Cross account.

If you already have an account, go to Step 2.

If you do not have an online account, you can create one.

(Note: You'll need to use an email address to create your online account. Be sure to use the same email address on your authorization form in step 2.)

Step 2. Complete authorization form

Either the member or representative needs to complete and submit the digital data disclosure and release authorization form.

Step 3. Revisit your third-party application

Your account should be available 2–3 business days after we have received the authorization form.

After we receive and process the form, the representative will be able to use third-party applications that support access to Blue Cross member data.
(Note: Third-party applications may need their own account setup to access them.)

You can then return to your desired third-party application to access the member's information.
