If you’re caring for a family member or friend, use these resources to help you manage his or her health plan and care.

Access health plan information

Even if you are a family member, you’ll need written permission from your loved one to get access to his or her health plan information.

For you to access health plan information from our files, including their eligibility, payment and claims information, your loved one will need to complete, sign and send the Authorization for disclosure of health information. You can choose from these options:

Help manage your loved one’s care

A Power of Attorney is a legal document that gives you the power to act in your loved one’s place. If your loved one ever becomes mentally incapacitated, you’ll need a Power of Attorney signed and filed with the state government to make decisions about your loved one’s medical care and finances.

Your loved one will need to complete and sign a Power of Attorney form in order for you to:

  • Make decisions about your loved one’s health care and finances
  • Sign papers on his or her behalf
  • Make a change to a loved one’s personal information, such as address or phone number

  • Minnesota Board on Aging — Offers free help to connect seniors to services in their community, including help with transportation, chore help, meals, housing options, legal services and more
  • Medicare.gov — Offers a resource kit to help you work effectively with Medicare to ensure family members and friends receive the best possible care
