Using your health plan is easy. Your national provider network, BlueCard PPO, works with doctors, hospitals and clinics across the country, including our onsite health center, True Care. Just show your member ID card whenever you get care.

Remember, if you receive services from out-of-network providers you will have reduced benefit coverage with our Enhanced and Standard HDHP plans. You will not have any benefit coverage for out-of-network providers other than emergency treatment if enrolled in the BCBS/Coupe Premium plan.

New for 2025: BCBS/Coupe Premium Plan

An important strategy to mitigate the rising cost of healthcare is to prioritize efficient usage and healthy outcomes in our medical plans. To this end, a new BCBS/Coupe Premium Plan will replace the current BCBS Premium Plan as of January 1, 2025.

  • This new plan is a variable copay plan which means that members will pay a flat copay for each covered service and prescription drug which can be viewed on the Coupe website before services are provided. There is no deductible or coinsurance on this plan.
  • The plan uses the same national provider network as our current BCBS plans and classifies providers into three tiers based on quality and efficiency of care. 
  • Tier 1 providers will have the lowest copay for services and the highest-quality outcomes based on Coupe’s research. 
  • The Coupe Health provider search tool makes it easy to find these high-quality providers, review applicable copays and make an informed decision before you receive services.

Additionally, Coupe will simplify billing by consolidating all medical and prescription copays into one monthly invoice for easier tracking and payment and eliminates confusing billing from providers.

If you enroll in this plan,  starting January 1, 2025, you will have 24/7 access to the Coupe Health Plan member portal at coupehealth.com .The member portal houses all your benefit information including provider, prescription and pharmacy lookup, bill statements, access to your Health Valet, and much more. The Coupe health mobile app will be available for download starting 1/1/25 via the App Store or on Google Play, visit the Coupe microsite at employers.coupehealth.com/allianz to learn more.

Pharmacy Benefits

All 3 medical plan options utilize the same covered drug list (KeyRx) and pharmacy network (Select) through Prime Therapeutics. The 2025 KeyRx Drug List can be found on this page under Prescription Drugs. You can search for in network pharmacies by selecting “Find a Pharmacy” on this page under Prescription Drugs (choose “Select Network”). After 1/1/25 you can view the pharmacy information specific to your plan by logging in to your plan’s member portal.

Your mental health is a priority

Learn how we're supporting your mental health below:

  1. Generic anti-depressant medications on the KeyRx Preventive Drug list are covered at 100%.
  2. Coverage for most mental health services obtained in the office or outpatient:
    1. Those who enroll in our Enhanced and Standard HDHPs pay no coinsurance after they meet the annual deductible.
    2. Those who enroll in the Coupe Health Premium plan will have 100% coverage, with no deductible or copay.
  3. Access to virtual mental health care through Doctor on Demand. Virtual visits are convenient and typically cost less than in in-person visit. Visit the Doctor on Demand site via the link on this page under Related Resources.
  4. On-site therapy visits are available at True Care in our Minneapolis campus for benefits-eligible employees and dependents regardless of if you are enrolled in a medical plan.
  5. Learn to Live is a self-paced online mental health program to help you and your family members (age 13+) manage stress, depression, social anxiety, insomnia or substance use. Activate your True Balance Experience to earn rewards today

By activating the Limeade ONE app, you will have quick access to fun activities, helpful tools and social support from your colleagues and Allianz – all designed to help you be your best.

When you (and your eligible spouse) participate in well-being activities, you earn rewards. If you are enrolled in a high deductible medical plan (Enhanced or Standard HDHP), you will receive rewards in the form of HSA contributions. If you are enrolled in the Premium Plan or are an employee not enrolled in an Allianz medical plan, you will receive rewards in the form of Recognition Central Points.

When you get emergency care or are treated by an out-of-network provider at an in-network hospital or ambulatory surgical center, you are protected by the "No Surprises Act," a provision under the Consolidated Appropriations Act (CAA) 2021.
