How to apply for a Minnesota Health Care Program

Minnesota Health Care Programs health plans from Blue Plus can help you pay for your health care if you can’t afford it or have a disability.

To enroll in a plan, you must first qualify for assistance from the Minnesota Department of Human Services. The application process depends on your age, if you have children and whether you have a disability.

If your eligibility changes, you have options

If you are no longer eligible for a Minnesota Health Care Program, you may get a health insurance plan directly from Blue Cross or through the MNsure website.

SecureBlueSM (HMO SNP) is a health plan that contracts with both Medicare and the Minnesota Medical Assistance (Medicaid) program to provide benefits of both programs to enrollees. Enrollment in SecureBlueSM (HMO SNP) depends on contract renewal.
