Your Blue Advantage Families and Children, MinnesotaCare, Minnesota Senior Care Plus or SecureBlue health insurance plan offers medical care. It also includes helpful programs, educational tools and support.

Please take 15 minutes to complete the online health assessment so we can connect you to programs that can help manage your care.

After completing the health risk assessment, you will:

• Receive a personal report that describes the impact of each risk factor on your health and provide you with an overall wellness score.
• Receive a summary report that you can share with your doctor at your next appointment.
